Position:  Full-time Dental Director (Leon Office)

Experience 1 - 2 years experience preferred

Education Current DMD or DDS (IA License)

Position:  Full-time Dentist (Leon Office)

Experience:  1 - 2 years experience preferred

Education:  Current DMD or DDS (IA License)


Position:  PRN (As Needed) Pharmacist to work in the Leon Office.

Experience:  Has at least 1 to 3 years of experience as a registered Pharmacist.

Education:  Pharmacist license for the State of Iowa.

Position: Full-time Dental Assistant in the Leon facility. 

Experience: 1 Year of Dental Assistant experience is preferred, but not required. 

Education: Must have current certification in radiology. Must have or be willing to obtain CPR and mandatory reporting certifications.


Position: Dental Assistant Chair Side Training Program

Summary: This program has been established to provide on the job training for the purpose of obtaining a certification as a dental assistant. 

Description: 1. CHCSI will follow the criteria and guidelines that are provided by State of Iowa Dental Board. 2. Training will be provided by staff dentist(s) and dental assistant(s) and hygienist(s). 3. Training will be completed within 12 months. 4. CHCSI will provide 100% of the expenses for training and testing. 5. Trainees will be required to sign a 2 year commitment to enroll into program. Commitment is to begin after certification is received.

Position: LMHC (Licensed Mental Health Counselor), LISW (Licensed Independent Social Worker), LMSW (Licensed Master Social Worker), or LMFT (Licensed Marriage and Family Therapist) to provide therapy services in our Leon, Osceola, Corydon, or Chariton offices.

Experience: Preference will be given to those with clinical experience, training in evidence-based practices for a variety of clinical presentations, and willingness to work with the full age range of clients. 

Qualified applicants will have a master’s degree from an accredited institution and hold a current LMHC, LISW, LMSW, or LMFT license.


Position: Full-Time Lab/X-Ray Technologist - Chariton Medical Facility

Experience: 1-2 years of experience preferred but not required.

Education: Applicants must be certified as a Radiology Technologist and willing to be trained in phlebotomy if not already.


Position: Care Coordinator (Chariton Medical Office)

Essential Duties: 
• Collaborate with treating providers in developing and implementing a care coordination plan to address treatment barriers for each low to moderate risk patient.
• Collaborative with partner organizations to promote care continuity and positive patient outcomes by maximizing efficiency of available resources/programs by helping patients connect to needed services.

Education & Experience: 
• BA or BS in a human service or similar field or CMA, LPN or RN nursing license
• Knowledge of, and experience in working with adults with chronic health conditions. 
• Case Management experience preferred. 

Position: Part Time Traveling Behavioral Health Facilitator 

Summary: Assist in coordinating communication between patients and ARNP/Psychiatrist ensuring consistent patient care and coordination of services. 

Experience: Qualified applicants must be well organized and computer literate. 

Education: High school diploma or GED. Preference will be given to candidates with CNA, CMA, or LPN license. Completion of Medication Manager class is preferred. 

Travel is required with this position to Centerville, Leon, Lamoni, Albia, Corydon, Chariton, and Osceola offices.


Position: Registered Nurse (RN)

Position Details:

Successful applicant will initially assist Director of Crisis Services in program implementation for the Crisis Access Center being constructed in Osceola. This position will eventually move to Osceola along with the Director of Crisis Services when the building is operational.


Current RN recognized by the State of Iowa Board of Nursing.

Knowledge of nursing care in accordance with regulations and practices established by the Iowa Board of Nursing.

Knowledge/familiarity with rules and regulations of Department of Inspections and Appeals, also the Division of Mental Health and Disability Services is beneficial.

Two years nursing experience and at least one year in behavioral or mental health services, preferred.


Position:  Part-time Housekeeper for the Leon facility. This position will work approximately 20-25 hours per week.

:  Professional cleaning experience is preferred, but not required.

:  High school diploma or GED.


Full-Time LPN or CMA to work in our Leon and Lamoni offices.

General Duties:
Greets and escorts patients to exam room for medical provider visit.
Obtains and documents subjective (chief complaint, allergies, pain) and objective (vital signs, weight) data accurately and concisely.
Applies nursing care in accordance with Chapter Six of the Code of Iowa.
Utilizes proper procedure with special consideration for patient safety.


Current LPN license recognized by the State of Iowa Board of Nursing or current CMA certificate.
Knowledge of nursing care in accordance with regulations and practices established by the Iowa Board of Nursing.
Two years nursing experience, preferred.

Position: Full-Time Billing Specialist - Leon Office

Experience: Medical coding knowledge and experience is preferred.

Summary: Responsible for daily operations of billing, to include: submitting claims for services; posting payments to patient accounts; processing denied claims; reviewing accounts receivable; and answering patient billing questions.

Education: High school diploma or GED
