Diane Bales, LMHC

Diane Bales, LMHC started working with CHCSI in December of 2019. She earned her Master's in Counseling from Capella University in 2009. Diane has worked in private practice since 2014 prior to joining CHCSI.  She specializes in the areas of depression, anxiety and trauma.  Diane also has experience working with victims of domestic violence and mental health issues with the elderly.  She currently provides services in our Osceola office. 

James Brooks, MD

Jim Brooks MD is a board certified psychiatrist, who joined CHCSI in June of 2017. He lives in Kauai, Hawaii, and works from home seeing patients via telepsychiatry. Dr Brooks graduated medical school from Wayne State University in Detroit in 1977, and did his Internship and residency training at the University of Colorado Medical Center in Denver from 1977-1982. He has extensive experience working with both inpatients and outpatients. Dr. Brooks currently serves patients in the Chariton Behavioral and Albia locations.

Kevin Comer, LMSW

Kevin Comer joined CHCSI in 2007. Mr. Comer earned his Master of Science from Drake University. Kevin serves as the Crisis Director for the organization.

Courtney Cook, MA, CRC, LMHC

Courtney Cook, MA, CRC, LMHC joined CHCSI in 2015. Mr. Cook earned his Masters in Rehabilitation Counseling from the University of Iowa. He is a Certified Rehabilitation Counselor and a Licensed Mental Health Counselor. He is currently providing services in our Centerville office.

Evelyn Conrad, LISW

Evelyn Conrad joined CHCSI in 2009. Ms. Conrad earned a Master of Social Work from the University of Maryland. She currently provides services to patients in the Lamoni office.

 

Beatriz Dantzler, LMFT

Beatriz Dantzler joined CHCSI in 2017. Beatriz has 30 years of experience in the field and has worked in UT, TX, CA, MO and FL. She has a wealth of experience in the field and is currently fulfilling her dream to work as a Licensed Marriage and Family Therapist. She was the Clinical Director for Recovery First and Recovery Institute of South Florida in Fort Lauderdale, and most recently held a position with Miami Courts directing their veterans program.  Beatriz sees children 12 and up, adolescents, adults, couples and families. She also specializes in chemical dependency and co-occurring disorders. Beatriz currently provides services in the Corydon facility.

Roshan Dasari, MD, MPH

 Roshan Dasari, MD, MPH joined CHCSI in 2009. Dr. Dasari earned his MD at BLDEA's Shri B.M.Patil Medical College in India and his Masters in Public Health, majoring in Epidemiology, from Tulane University School of Public Health and Tropical Medicine, New Orleans, LA. Dr. Dasari completed his Psychiatry Residency at the University of Kansas Medical Center, Kansas City, KS. He is certified by the American Board of Psychiatry & Neurology and currently provides behavioral services via telemedicine in our Albia, Centerville, Chariton Behavioral, and Corydon offices.

Richard Davenport, LMHC, LMSW

Richard Davenport joined CHCSI in 2014. Mr. Davenport earned his Master of Science in Counseling from the Drake University. He currently provides behavioral health services to patients in the Leon and Osceola offices.

Rebecca DeVine, PMHNP, DNP

  Rebecca DeVine started working with CHCSI in 2015. Ms. DeVine earned her Baccalaureate of Science in Nursing from Morningside College in Sioux City in 2008 and also earned her Doctor of Nursing Practice from University of Iowa in 2015. Ms. DeVine is also a certified Psychiatric Mental Health Nurse Practitioner.  She currently provides psychiatric services in the Albia, Centerville, Osceola, Corydon, Leon, and Lamoni offices.

Matthew Dzak, Psy.D, LPC

Matthew Dzak, PsyD, LMHC, LPC joined CHCSI in 2015. He earned his Doctorate in Clinical Psychology from the Forest Institute of Professional Psychology in Springfield, Missouri. Dr. Dzak is a Licensed Clinical Psychologist and is credentialed with the National Register of Health Service Psychologists. He offers a variety of psychological assessment services for children, adolescents, and adults. He also provides therapy services for adolescents and adults. He see patients in the Centerville office and serves as the Assistant Behavioral Health Director for the organization. 

Jenifer Grissom, FNP, ARNP, PMHNP

Jenifer Grissom joined CHCSI  in 2015. Her Masters of Nursing was earned at the Clarkson College in Omaha Nebraska. She is also a board certified as FNP (Family Nurse Practitioner) and PMHNP (Psychiatric Mental Health Nurse Practitioner). Jenifer sees patients 10 and up at the Osceola Access Center.

Dennis Hartman, CADC

Dennis Hartman joined CHCSI in 2010. Mr. Hartman earned his Bachelor of Science from Iowa State University. He currently provides substance abuse counseling and serves patients in the Centerville and Corydon offices and also serves as the Substance Abuse Program Director.

Don Henderson, LMHC

Don Henderson started working with CHCSI in November 2019. He earned his first Master's in Trauma Counseling from Calvary Seminary in Kansas City, Mo.in 1996. He earned his second Master's from Liberty University in 2013 in Professional Counseling. Don completed his Internship with Bay-Arenac Behavioral Health in Bay City, MI working in the ACT program. He has extensive background working with disease trauma and depression. The past few years, Don has worked in school-based programs in Central Iowa schools and on the Apache Indian Reservation in San Carlos, Arizona. Don works with clients 10 years-old and older covering a wide range of Mental/Behavioral issues. He currently provides services in our Osceola office.

Carol Hubbard, MA, LPC, RPT-S

 Carol Hubbard, MA, LPC, RPT-S joined CHCSI in 2018.  Ms. Hubbard earned her Masters in Counseling from Liberty University in Virginia.  She received her post graduate certificate in Play Therapy from Mid America Nazarene University in Olathe, KS.  She is currently providing services in our Princeton office and will be working in the surrounding schools.

Krista Johnson, PMHNP-BC

Krista Johnson PMHNP-BC joined CHCSI in 2019. She earned her MSN from Walden University in 2014 and her Post Masters Certificate from Allen College in 2019. She is a board certified Psychiatric Mental Health Nurse Practitioner. Krista currently provides in-person and telehealth psychiatric services in the Albia, Centerville, Chariton, and Corydon offices.

Chelsey Joiner, CADC

Chelsey Joiner, CADC, joined CHCSI in 2018. She earned her Bachelors Degree in Human Services with a Minor in Trauma Advocacy from Buena Vista University. She is currently working on her Masters in Mental Health Counseling from the Townsend Institute Concordia University, California and projected to be completed in late 2021. She believes in meeting people where they are at and being real for transformative change. She currently provides substance abuse counseling and serves patients in the Osceola office. 

Hiedi Lane ARNP, FNP-BC, PMHNP-BC

Hiedi Lane ARNP, FNP-BC, PMHNP-BC joined CHCSI in 2017.   She earned her BSN from the University of Iowa and a Masters and Post Masters Certificate from Northern Kentucky University and is a  dual certified Psychiatric and Family Nurse Practitioner.  She provides services to patients 10 and over in the Centerville office.

Shobhit Negi, MD

Dr. Shobhit Negi started working with CHCSI in 2015. Dr. Negi earned his MD from King George's Medical College in Lucknow, India in 1996. Dr. Negi did a residency at Perleman School of Medicine in Forensic Psychiatry, residency at the University of Maryland in Child and Adolescent Psychiatry, a residency at Howard University Hospital in Psychiatry, and a residency at Sarojini Naidu Medical College in Psychiatry.  He currently serves patients in the Centerville, Leon, and Osceola offices.

Rachel Pobanz, Psy.D., joined CHCSI in 2017. Dr. Pobanz earned her Doctorate in Clinical Psychology from the Illinois School of Professional Psychology in Schaumburg, Illinois. She is a Licensed Clinical Psychologist in Iowa and is credentialed with the National Register of Health Service Psychologists. She is currently providing psychological services, including therapy and psychological evaluations, in our Albia office.

Colleen Stephens, LISW

Colleen Stephens joined CHCSI in 2019 after serving over 24 years in the United States Army as an active duty Social Work Officer. She has worked in Honduras, Iraq, NM, OH, TX, KS, LA, and IA. She graduated in 1998 with a Masters of Social Work from New Mexico Highlands University and has completed over 3 years toward her Ph.D. at the University of Kansas. She has clinical expertise in Crisis Response, Corrections, Domestic Violence, Sexual Assault, Chemical Abuse and Addiction, Traumatic Brain Injury, Warrior Transition, PTSD, and inpatient/outpatient Mental Health. She currently provides behavioral health services to adults at the Centerville and Corydon offices.
