Services Provided: Child Health Specialty Clinics (CHSC) is Iowa’s Title V Program (of the Social Security Act) for Children with Special Health Care Needs. The Creston Regional Center serves Taylor, Ringgold, Decatur, Clarke, Union, Adams, Adair and Montgomery counties. We presently provide the following services to children and their families –
  • Integrated Evaluation and Planning Clinics (IEPC) – It is staffed by pediatrician, pediatric nurse practitioner, dietitian, psychologist and social worker. This consultative clinic provides comprehensive evaluations and a multidisciplinary plan of recommendation. Examples of concerns that children are commonly referred for include: emotional/behavioral issues, educational/behavioral issues, attentional or hyperactivity concerns, developmental delays, growth, encopresis, obesity and other medical concerns.
  • CHSC Services for Birth to Five – Assessment/evaluation of physical and neurological status, development and growth, Parent support and targeted education on parenting, Nutrition evaluation and/or nutrition consultation, Care Coordination as appropriate.
  • Behavioral Health Program (BHP) – Provides intensive care-coordination services as well as additional educational, psychiatric, and psychological consults for children with mental or behavioral health concerns.
  • Nutritional Consults – Serve children birth to 21 years, Dietitians trained in nutrition considerations for children with special health care needs consult on growth concerns and make dietary recommendations.
  • Home and Community Care Planning – Provides family-centered care coordination services for children and youth with special health care needs.
