Position:  LISW (Licensed Independent Social Worker), LMHC (Licensed Mental Health Counselor), LMSW (Licensed Master Social Worker), LMFT (Licensed Marriage and Family Therapist), or Licensed Psychologist (P.D./Psy.D.) to provide behavioral health services in our Leon or Lamoni offices.

Experience:  Clinical experience and a willingness to work with children and adolescents are preferred.

Education 

Qualified applicants must hold a current LISW, LMSW, LMHC, or Psychology license (or be license-eligible) and be trained in the delivery of evidence-based best practices. Qualified Ph.D./Psy.D. applicants must have graduated from an APA-accredited institution.

Position
: IHH Nurse Care Manager (Based in Centerville with possible travel to Corydon, Chariton, Albia, Leon and Lamoni)

General Duties:

Work with Integrated Health Home patients on a specific plan to support overall wellness.
Work with Doctors and Behavioral Health Providers to help improve overall physical and behavioral Health.
Assist with tracking and gathering of relevant health information to support IHH initiatives.
Assist in developing care plan for individuals with high medical needs. 

Education/Experience:

Current RN or BSN License recognized by the State of Iowa Board of Nursing.
Knowledge of nursing care in accordance with regulations and practices established by the Iowa Board of Nursing.

: LPN/CMA (Lamoni Office)

General Duties:
Obtains and documents subjective (chief complaint, allergies, pain) and objective (vital signs, weight) data accurately and concisely. 
Applies nursing care in accordance with Chapter Six of the Code of Iowa. 
Utilizes proper procedure with special consideration for patient safety.


Education/Experience:
Current LPN License recognized by the State of Iowa Board of Nursing or current CMA certificate. 
Knowledge of nursing care in accordance with regulations and practices established by the Iowa Board of Nursing. 
Two years nursing experience, preferred.

Position: Technology Support Specialist

General Duties:  Ensure proper computer operations so that CHCSI staff can accomplish organizational tasks. This includes receiving, prioritizing, documenting and actively resolving all staff help requests. Problem resolution may involve the use of diagnostics and help request tracking tools, as well as require that the individual give hands-on help at the desktop level.

Education/Experience: 
Associate degree in an IT field
Experience supporting Windows PC’s and Microsoft Office
Excellent communication skills
Must be able to work flexible hours with some travel
Must be able to work in a fast paced technical environment
Must have a good technical aptitude and a quick learner

Position: Patient and Community Education Specialist (Leon Office)

Requirements: Applicants must be incredibly organized and able to work independently. This position will be responsible for coordinating/providing community education events, as well as providing patient education on an individual basis. Applicants must be willing to travel to all locations. 

Education/Experience: Associates degree in health or human services related field preferred, but not required.

General Duties:  Responsible for the oversight of all reception processes, procedures, and performance, to include: patient scheduling, patient registration, and overall customer service. Travel to all sites is required.

Education/Experience: Minimum of Associates Degree or equivalent.

Position: Chronic Care Health Home Coordinator (Leon Office)

General Duties:  
Collaborate with Health Home Team and treating providers in developing and implementing a care coordination plan to address treatment barriers for each low to moderate risk patient.
Collaborative with partner organizations to promote care continuity and positive patient outcomes by maximizing efficiency of available resources/programs by helping patients connect to needed services.


Education & Experience:
BA or BS Degree in human service or similar field required.
Knowledge of, and experience in working with adults with chronic health conditions.
Case Management experience preferred.
